• Communication regarding Scholarship, SMP, and other beneficiaries and partners (Initial interview, home visitation, MOA signing)
• Regular Monitoring and Assessment of beneficiaries
• Case/Status Updates every School/Home visit
• Database and Profile of beneficiaries and partners (Schools, Parishes, Community Partners)
• Documentation of seminars and trainings through sharing of personal experiences

Personal Data primarily collected by APTSFI may include the following, among others: your personal information; e-mail address; other contact details such as your address; your IP address; your social media account; and/or any other information relevant to the communication you have sent or provided.

1. Use of coding system or case number.
2. All Beneficiaries and other Programs Folders shall be marked “CONFIDENTIAL”.
3. Filing Cabinets for confidential records are properly marked as “UNAUTHORIZED PERSON. DO NOT OPEN”.
4. Only the social workers are the authorized personnel who can access the filing cabinets of the beneficiaries and program folders with the permission of Supervising Social Worker.
5. Communication/Consent from the beneficiaries/partners before dissemination of information outside the APTSFI.

HOW LONG WILL APTSFI RETAIN YOUR INFORMATION

APTSFI will retain your Personal Data for as long as necessary to fulfill the purposes outlined in this Privacy Statement and communicated to you, unless a longer period is allowed or required by law. Unneeded records, documents or old files is subject for assessment of Social Workers before shredding.
